Fingers stuck in bicycle chain answer

1) X Ray finding
Complete amputation of distal phalynx and distal 1/2 intermediate phalynx of the 4th left finger



2) vital sign normal and patient is not in shock
Normal RR (16-30)
Normal HR (60-120)
Normal BP (94-109/56-59)


3) Management for this patient
a) control the bleeding
b) analgesic (IV Tramadol)
c) Irragation and debridement under fentanyl
d) re attachment of the hand if possible

notes:

this patient finger was severely damage. this problem is often seen in patient who is injured due to stuck in bicycle chain. the reason is the cut cause muscle damage and un adequate tissue. plus, the bleed site is very dirty due to oil.
